Brief Answer:
Needs further evaluation

Detailed Answer:
Hello,
I've reviewed the reports...

The cause of your symptoms seems like low hemoglobin. Now, priority is to find the cause for low hemoglobin and low RBC count. One of the contributing cause is iron deficiency. But you should get Folic acid levels done and if these are normal then you'll need bone marrow biopsy to find out the cause.

Anyway, you should start yourself on iron supplements like Tab Fericip XT twice a day after meals to be continued. It's better to get a Hematologist's opinion on this.

For low Vit D, you should have Tab Uprise D 3 60,000 international units once a week for eight weeks, then once a month for six months.

For thyroid problem, you should be on Tab Eltroxin 25 microgram once a day before breakfast. All these are prescription based medications.

Hope this helps you and get back if you have any doubts.

Regards
