Question: 57 y/o man with type 2 diabetes, history of alcohol abuse, smoking and drug abuse, currently experiencing very swollen lower legs, shortness of breath at work, and having to prop up in bed to be able to breath.

The symptoms you described are suggestive for cardiac or renal disease.
You need to do some blood tests and a cardiac ultrasound, except of a detailed clinical examination;
The blood tests needed;
-Full blood count
-D dimer
-Hba1c
-AST, ALT, GGT
-Total proteins and albumin in blood as well as in the urine.
-TSH
-BUN and creatinine
-Electrolites
-Amilase and lipase.
