Hi. Does he have any other illnesses that he is on medication for? Does he have any swelling in his abdomen?
If not then it could simply be Hydrocele which is a fluid collection surrounding the testicles. It is usually a painless condition and causes mild discomfort.
You should consult a Surgeon who can examine the patient and advice accordingly. He also might require an Ultrasound to get to a diagnosis. But you need not worry. It can be treated easily.
