Brief Answer:
take him to emergency

Detailed Answer:
Hello,
He could have a testicular torsion. This is a twisting of the testicle on the cords and is very serious. It of course could be from other causes but he needs to see a doctor as soon as possible because if it is torsion and it is not corrected soon he could lose that testicle. Please get him seen immediately

Brief Answer:
yes

Detailed Answer:
Testicular torsion when it is complete is very painful. Still he needs to be checked. If he is in no pain at all when sitting or lying down it can probably wait until tomorrow. My concern for torsion was increased when you said that the ibuprofen did not help. Most causes of testicular pain would be improved with ibuprofen but torsion would not.
