Posted on Tue, 21 Jul 2015
Question: My wife, 32 yrs old had C-section last month. In between she had pain just above incision line and also mild fever. The Gynachologist prescribed Combiflam (paracetamol+ iboprufen) tablets. The pain disappeared but next week she started filling pain during urination also had mild fever. Ultra sonography revealed irregular focal thickness (3.2x1.2 cm) in urinary bladder (basal part). Urin routine analysis does not show any RBC or pus cell. Other parameters are within normal range. Full abdoaminal songraphy show all other organs witin normal limit. Plaease suggest me posssible cause of thickening and nature of thickness. It would be further helpful on suggesting preventive treatment.

Brief Answer:
focal thickness is due to various reasons

Detailed Answer:
Hello dear, welcome to healthcare magic.I understand your concern.
In my opinion the focal bladder wall thickening might be due to various causes like:-
1)Cystitis-bladder infection.
2)Presence of any blood clot or pus.
3)Any malignancy.
As there is fever and pain during urination the infection is the first thing to be ruled out which is already done.
Better to go for a course of antibiotics on empirical therapy and see.So discuss regarding that with your doctor.
The accurate diagnosis can be known by CT scan of urinary bladder or cystoscopy.
Meanwhile adequate fluid intake is advised.Coconut water and cranberry juice might also help in decreasing urinary symptoms.
Hope this satisfies your query.
Feel free to discuss further.

Dear Doctor,
Thanks for your reply. I would like to inform further about the latest status.
Based on the Sonography & Urine report, Urologist suggested to take Meftal spas for five days. At present, she is taking 1 tab per day that siginificantly reduced pain and also no fever since last five days. Is it a sign of reducing the problem? However, on yesterday, urologist suggested to go for Cystoscopy. I would also like to mention, she felt severe pain while removing catheter after C-section. Whether it had been a kind of Heamatoma?
Should we continue with Meftal spas and wait for some days before taking up for Cystoscopy.

Brief Answer:
cystoscopy might be helpful

Detailed Answer:
Hello dear,I got your query.I understand your concern.
In my opinion the meftal spas tablet is a pain killer due to which the pain is decreased but not for decreasing the bladder thickness. It is just to decrease the symptoms but not the cause.
So better diagnosis and treatment might be possible with further investigations like cystoscopy or CT.
The catheterization during pregnancy might cause infection mostly rather than haematoma.
Continue the meftal spas and it might be better if you go for cystoscopy. As if at all any abnormality detected the treatment might be given at earlier stage.
Hope this satisfies your query.
Feel free to discuss further.

Thanks Doctor,
Hearty thanks,

Today we have consulted to another urologist. As thre is relief from pain and fever, he suggested to get urine culture and urine cytology done. The onsite sonography as per his view does not show significant thickening and asked to wait till the tets reports are ready. According to him it may be infectious imflammation rather than tissue growth. However, he is also interested to d cystoscopy at the later stage. Currently he prescribed Amoxiciline 500mg thrice a day and revisit after three days with urin reports.

Will come back to you for any suggestions after the reports are in hand.

Regards.
