Brief Answer:
Different possible causes.

Detailed Answer:

Hi,

The tightness of the throat or chest along with increased heartbeat can be due to different reasons.

With a history of diabetes, it is usually best to have it assessed unless there is an obvious cause.

Heart disease, though it is not the most common reason, needs to be ruled out.

Causes other than heart disease include--
(1) anxiety
(2) chest infection eg pneumonia
(3) trauma
(4) the side effect of medication
(5) increased exertion

If none of these is likely or previously diagnosed, then it is best to be seen by your doctor for assessment which should include a physical examination and an ECG to look at the rate and rhythm of the heartbeat.

I hope this helps.
