DYSPEPSIA /GERD or
ESOPHAGUS or

I would advise a PPI such as esomeprazole and a PROKINETIC like domeperidone and an anti spasmodic like hyoscyamine for a week. An ECG should be done in the ER if the tightness feeling gets worse.
