Warm Welcomes. I appreciate your trust in HCM and assure you are on best portal on the globe for expert online medical services.
I have thoroughly reviewed health query you raised, and well realize all of those concerns. Being your physician, I will take care all of them today and will extend my expertise to solve the problems.
Tremors in hands, painful weak muscles of extremities, frequent falls, loss of memory and hand grips may have many reasons with some of the overlapping symptoms.
These may be Thyroid disease; Chronic Hypertension; Fluid and electrolyte imbalance, nutritional deficiencies (vitamin deficiency, calcium and magnesium loss). Loss of function of the kidneys and heart is also a cause of weakness in muscles of legs.
You need to consult an Endocrinologist to see the status of your Thyroid gland. It is mandatory for you to go for serial Thyroid profile testing as to see how is it working. Whole of the body functions depends on it. If there is a disease, we may see tremors. Also he will have to rule out normal functioning Parathyroid glands for that is also important for keeping a calcium balance in body (between blood, muscles, teeth and bones).
Vitamin B12 and Vitamin D deficiency also impacts on bodily functions. Need is to test their levels. Your Primary care physician will help you in this regard. Also ask him to test your blood pressure as high blood pressure is one of the major causes for your symptoms. Your Physician will also test you for serum electrolytes and mineral levels.
Ask your Physician to go for a thorough heart evaluation as pump failure of the heart is also a major cause for symptoms you have as a failing heart is unable to pump properly and this causes lack of nutrient supply to the muscles of legs and hands.
Once work up is complete, diagnosis can be made and based on that treatment goes accordingly.

Welcome back with a follow up. Thanks for adding details here and I am happy to assist you in this regard.
Yes, it is still possible that you may have issues even being on medications. Taking medications never promises that a patient and nonpatient will be same. Disease and its side effects on the body still remains there even if the disease is well under control.
You need to consult your treating Physician and an Endocrinologist for thorough evaluation of the disease status and review of medications you are on.
