Hi.....you are right, its stress ulcer or aphthous ulcer over the tongue. The bumps outside look like a viral exanthem or a viral illness induced rash. You neednot worry if he's active and his usual self. Will suggest you danger signs or red flags if fever and diarrhea, so that you can be on guard.
If your kid is normal in between fever episodes in a single day...I don't think you should worry as its only 3rd day of fever. Danger signs include breathlessness/ rash/ low urine output/ seizures/ unconsciousness/ blood in motion. Don't worry if these are not there & the kid is active.
He also seems to bw having acute watery diarrhea. Unless the kid's having low urine output or very dull or excessively sleepy or blood in motion or green bilious vomiting...you need not worry. I suggest you use zinc supplements & ORS...

Hi...If he has diarrhea and still urinating well - that means he is not having dehydration.
Fever of 100.9 doesn't warrant Paracetamol. Paracetamol can be given in the dose of 15mg/kg/dose every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

Hi...you have mentioned that the bumps are increasing in numbers. I agree that IRS an important sign. But an image and an information on the general well being of the kid will be immensely helpful.
I suggest that you see your pediatrician, but if there's no fever and the kid is active otherwise, I don't think antibiotics would be needed.
