Brief Answer:
Some more tests are necessary for sure.

Detailed Answer:
Hello!

Thank you for the query.

As you have had abdominal ultrasound and blood work, the next step would be liver tests (AST,ALT, GGTP, AP, bilirubin), pancreas tests (amylase, urine amylase), stool test and stool culture test. Any issue in the pancreas/bile ducts can give such pain.

Such symptoms can be sometimes caused by chronic bacterial infection. Especially if you have this symptoms for some time now (not just few days). If so, this symptoms could appear after antibiotic intake or some travel (like abroad).
If you find the infection possible, you should try Rifaximine treatment and start probiotic intake.

Upper right abdomen is where a part of large intestine is present. And you are close to 50. This makes colonoscopy the test to be done if nothing is found in the tests mentioned above. In some cases large intestine polyp or tumor can act like that and give similar symptoms.

Hope this will help. Feel free to ask further questions.
Regards.
