What Causes Upper Right Abdominal Pain While Suffering From Crohns Disease?

Question: Hi - i have Crohns disease. I had a colonoscopy done a few days ago. Lower colon check indicated mostly fine except a likely non cancerous growth - a sample was taken. I have extreme pain on upper right side. I had vomiting / diarreha about a week ago. Diarreha sort of remains but vomiting has stopped. Gastro wants to do MRI of upper intestines.

My appetite is limited these days. The pain is severe especially hours after eating.

Which particular type of crohns does this sound?

I read carefully all your concerns and I understood your query.

You are reporting to have experienced diarrhea with vomiting, which are suggestive for flare up episodes of Crohns disease. On the other hand you have done lower colonoscopy and it was normal. This endoscopic finding doesn't explain your symptoms.

Since Crohns disease isn't limited in colon but may involve other part of GI tract, you need to evaluate small bowel too. The best way to evaluate small intestine (bowel) are
-     Abdominal MRI or
-     Capsule endoscopy.

This is why I agree with your doctor and at this stage I would have recommended doing MRI in order to evaluate small bowels (intestine).

Moreover other causes that may explain your symptoms (and need to be evaluated) are gallbladder / pancreatic issues, which are better diagnosed with abdominal MRI.

I agree a MRI is warranted. Should i be even more alarmed when i pee and i see a bit faint of blood when i wipe?

You are reporting to have a bit faint of blood after urination; If you are sure that it comes from urinary tract and not from anal area than it can be related with :
- urinary infection
- stones, sludge in bladder or urinary tract

If I were your treating doctor I would recommended to do
- urinary analysis
- urinary tract ultrasound.

Meantime I suggest to:
- use plenty of water
- use chamomile tea to wash genitourinary region

Hi there - sorry the blood isnt from the urinary tract but rather from the anal with mucus. I feel bloated with swelling in upper abdominal and back pain.

Thank you for providing more of your medical data.

You are reporting to have blood with mucus from anal area and this is e typical sign of Crohn disease

Otherwise another reason of red blood in stools with mucous is anal fissure or hemorrhoids. Both of them are associated with anal pain during defecation. If you have these symptoms too,you can start to use preparation H in anal area.

With regards to bloated , abdominal swelling it can be explain as well from Crohn disease.
To my opinion except colonoscopy you should perform and PCR
test. It will help to confirm inflammation process and Crohn disease flare up.

If PCR level is high than you should start immediately the treatment for Crohn disease with cortisone , mesalasine or/ and antibiotic treatment.
