Brief Answer:
Clinical evaluation and investigations

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/67 - discontinued Motrin and any type of pain medicines after the pain started - severe pain upper right that *fluctuates - from no pain the severe without any notice - (sudden/spasmodic?) - below the rib cage - **radiate to the back - started with headache which you never had in past - chills - scratchy throat - slight hoarseness -

My thoughts:
The pain in the right upper abdomen under the rib cage and is spasmodic with reference to the back can be due to:
Gastric ulcer on the posterior wall.
Pancreatitis
Stones or obstruction in the bile ducts with secondary problems.

Headache with chills and scratchy throat is indicative of flu or can be secondary to the infection of the systems just talked about.

To get a proper diagnosis and a plan of management:
Ultrasonography of the abdomen/ CT scan of the whole abdomen.
Tests of blood: CBC, amylase and lipase for pancreas, Liver function tests.
Upper GI Endoscopy
Stool: routine - occult blood
Urine
Clinical evaluation by a Doctor and preferably a General Surgeon.
Rush to ER for clinical evaluation, investigations, admission if required and proper treatment as per the findings:
EKG on a safer side.

Further treatment will depend upon the findings of clinical evaluation and actual report. I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication...
