Question: What causes 16 ounce urine retention, with an adequate urine flow stream, when the prostate enlargement has been reduced and what is the cure?

Brief Answer: It is Chronic Retention of urine due to BEP. Detailed Answer: Hello Thanks for your query,based on the facts that you have posted it appears that you have retention of urine of 16 ounce (1000) ml. You might have enlarged prostate since long time with obstructive urinary symptoms with significant residual urine in bladder.What you assume adequate urine flow is retention with overflow . In enlarged prostate at initial stage bladder tries to pump out the urine by increased frequency of passing urine .However Over a period of long time bladder muscles get thickened due to overload and gradually gets trabaculated and bladder mucosa gets pitted in to these trabaculations causing saculation and finally multiple large saculations are being formed called diverticulum are formed in bladder. This results into weakening of bladder power that causes painless retention of urine which is called as Chronic retention of urine, so that patient does not get pain or sensation of bladder being full of urine even though large quantity of urine gets collected in bladder. Please consult qualified Urologist for clinical assessment and revaluation by doing investigations like 1) Ultrasound scanning of the abdomen to assess the size of the prostate and post void urine volume. 2) Cystometry and Uroflowmetry to asses strength of bladder musculature and urine flow per second. Further treatment will depend upon the results of these tests . You may need urinary diversion by doing Supra Pubic Cystostomy so that bladder will get drained continuously and will get complete rest .This will help bladder to regain power which may take 6-8 months ,so that appropriate surgical procedure can be carried out at later date provided you are fit to undergo surgery under epidural anaesthesia.
