The infection was cured after treated by doc by medicines listed below .. but after few days it reappears ..
first my penis foreskin n underskin starts to get white dead skin like layers .. then when I get erections the foreskin kind of ruptures and the fissures are formed as shown in the pic attached..
What is this exactly?

I have gone through your query and I have also reviewed the Image.
I can see vertical cracks, fissures all round the circumference of prepuce/ foreskin. This is classical of Candidal Balanoposthitis.
Repeated candidal infections of glans and prepuce are common in diabetes patients and since you are a known diabetic therefore probably this is the reason for recurrence of candidal infection. This condition is usually itchy.
No, this i not a sign of HIV/ AIDS. This is not related to masturbation.
If I was the treating doctor I would treat this with a single dose of an Oral antifungal e.g Tablet Fluconazole.
Topically I would ask you to apply an antifungal cream e.g clotrimazole 1% cream preferably which also contains a mild topical steroid e.g beclomethasone dipropionate 0.025%, twice daily for 7 days.
You may clean this area with plain water. Avoid soap in this area.
The underlying cause i.e raised blood sugar must be regulated to levels within normal range Or else candidal balanoposthitis may recur.
Therefore I suggest that you get your blood sugar levels checked and take appropriate treatment if your sugar levels are raised above the normal range.

should I use only PLAIN cool or XXXXXXX warm water (without any kind of soap not even DOVE) even when the infection is cured for 365 days .. or only till the fissures are visible ??

Use lukewarm plain water on all 365 days to clean this area. None of the soaps are not meant to be used on the delicate genital skin.
