Brief Answer:
no need to take it further.

Detailed Answer:
Hello dear, thanks for your question on HCM.
Treatment of tuberculosis (any organ) should not given more than 6-8 months.
So you should stop the treatment.
And ideally , four drugs are needed for only first 2-3 months of illness (intensive phase). No need to take four drugs for entire treatment duration.
AKT 4 is having four anti TB drugs. Rifampicin, ethambutol, isoniazide and ethambutol.
These four drugs should be given in first 2-3 months of treatment. And then two drugs (rifampicin and isoniazide) should be continued for remaining 6-7 months.
It seems you have taken improper treatment.
So no need to take further treatment, if your neck swelling is reduced or disappeared.
Ethambutol can cause visual impairment due to optic nerve damage. So you may have ethambutol toxicity as a cause for your vision problem.
So consult ophthalmologist and rule out ethambutol toxicity.

Hope I have solved your query.
I will be happy to help you further.
Wish you good health.
Thanks.

Brief Answer:
get done chest x ray first.

Detailed Answer:
Hello dear, thanks for your follow up question.
No need to take TB treatment further as your swelling is reduced in size.
And don't take rifampicin and isoniazide for cough. First get done chest x ray to rule out tuberculosis in lungs. Don't take it empirically.
Hope I have solved your query.
I will be happy to help you further.
Wish you good health.
