What Causes Weakness Along With Swelling On Legs And High Bilrubin Level?

Question: Hello

Current Symptoms
Feeling weak
Leg swelling
Less urine
symptoms of jaundice ( total Bilrubin 5.2)
pimples on face

History
Initially My mother(pure veg and Non Alcholic) faced problem swelling in legs around February of this year 2014.
we thought that she is having strain because of too much travelling.
In the Month of may, she came to visit me in XXXXXXX & did a master checkup and
listed are the results of the test
liver function test came abnormal
Ultra sound-normal
Color Doppler - normal
ECHO- Normal
Thyroid profile _Normal
Hepatitis profile - normal
ASMA/ANA - Normal
ECG- Normal
Doctor Said us that she is having liver problem
we started medication for her and afterwards she was feeling better in a month.

After 2 months, she started taking Ayurvedic medicine from our family doctor
Initiallay she was felling very better (almost no swelling )
Again from last 2 weeks back ( 11 NOV) swelling in leg started and feeling weak.
Doctor asked us to do copper urine 24 hr test (result =54Ug) attached report of results

please find the attached reports accordingly and prescription.
Ultra Sound ,Colour Doppler ,ASMA/ANA,LFT INR test ,copper urine test -24 Hr ,prescription from doctor
Note : From 17th Nov She started using Udliv tablet

1)please suggest us with next step .
2)My Doctor said it is because of copper or Ayurvedic medicine she has taken . what can be done to improve .will the prescription be enough?
3)what diet to follow ( like special food like turmeric , bottle gourd , Nuts etc) for her ,currently having no salts at all, she is having muscle cramps in night
4) IS the condition reversible for normal functioning

Thanks for the query

I have gone through the blood tests you have done in detail and here are my interpretations

1. The liver dysfunction is not due to copper metabolism as the urine copper excretion is within normal limits and also because the eye examination is negative for the KF ring. The liver disease arising due to improper copper metabolism is known as Wilson's disease

2. Drug induced ( ayurvedic drugs) hepatitis is possible. The only way to know is to stop the drug and see for improvement. Continue to take Tab Udiliv.

3.I recommend you to recheck the liver function after 2 weeks, if the liver function does not improve please approach your gastro enterologist for a liver biopsy, it is gold standard for diagnosing any liver dysfunction
4. I advice my patients to follow food rich in carbohydrates. Like rice, chapathi, etc. Since the serum albumin is on the lower side I recommend her to take B-Protein powder 2 tsp with milk twice daily. Avoid food rich in fat like fried items, cheese, butter, etc
5. Muscle cramps are not related to the liver dysfunction, most muscle cramps are because of dehydration, please ask her to drink atleast 2.5 litres of water per day

It is most likely to be reversible as there are no signs of chronicity like cirrhosis on ultra sound or varices in the endoscopy. To know for sure she has to undergo biopsy of the liver.
