Brief Answer:
Ultrasonography and FNAC of nodes

Detailed Answer:
Hi.
Thanks for your query.
Noted the history, investigations, Medications and understood the concerns.
Please give additional information for me to assist you better:
Since when are the symptoms present?
What are the sizes of lymph nodes? Are they painful or tender? How is the skin over it?
Please post photos of tongue lesions and of the lymph nodes if can be seen.

All the symptoms together indicate that there is an infection.
Hence I would advise the following additional investigations to explore more about a proper diagnosis:
Ultrasonography of the nodes and FNAC that is fine needle aspiration cytology.
Blood culture and sensitivity sometimes helps a lot.
Since you are sensitive to Ibuprofen you have to take another anti-inflammatory medication as would be advised by your Doctor.
Antibiotic cover if there are any signs/symptoms or investigations reports indicating infection.
Multivitamins since there is fatigue.
About Insomnia your treating Doctor would advise since you are already taking medications like Olanzipine and Amitriptyline.
I hope this answer helps you.

The symptoms have been ongoing for a year do you think it’s related to hiv or has that been ruled out as tests are negative. There are no lesions on tongue currently and lymph nodes are slightly throbbing and no redness around the skin just a little itchy
Bare in mind my encounter was over 25 years ago and currently living with one partner for the past 14 years who has also tested negative

Brief Answer:
Tests for more

Detailed Answer:
hiv 4th gen tests negative x3 as noted by you hence does not look to be due to HIV if the tests are recently done - 3 months later than the last exposure.
Hence go for the above tests to get a proper diagnosis and plan of management.
