Brown patches on the face can be due to aging process or you have been exposed to strong XXXXXXX weather for long times. Liver problems should be also considered as you are a heavy smoker. Liver function test and abdominal ultrasound is then required.

The while patches seems to be related to vitiligo from your description; however the photo will help specify it accordingly.
