Symptomatically the white marks you see on the anal mucosa could be due to minor cuts/trauma. Tenesmus and the straining may be adding to the problem.

For now, you can apply topical lignocaine jelly over the area about 20 minutes before passing stools. I would suggest you to take plenty of fluids orally and high fiber diet to help in easy passing of stools and for the cut mucosa to heal faster. If it hurts, warm sitz bath will be comforting. And for stomach cramps you would need anti-spasmodics like Dicylomine.

It would be better to get it examined by your primary physician once to rule out any underlying medical conditions. You may need a stool examination.
