I am Dr. Kakkar. I have gone through your query and I have also seen the attached photograph.
I can see a folliculitis lesion in the photograph with a pus point. It must be painful
-I would like to know the duration of this?Is folliculitis recurrent?
-Which antibiotic or antibiotics have you tried? do these lesions respond to antibiotics? do they recur again after you stop antibiotics?
I have reviewed your discussion with another dermatologist on healthcaremagic in which you are being advised for dandruff and scalp itching (seborrheic dermatitis). -How severe is your dandruff?
-I hope you are using a specific shampoo for dandruff and itching? How has the response been to these? Are they totally ineffective or they seem to benefit?
Itchy scalp bumps can be fungal in origin rather than bacterial. Malassezia furfur, a yeast commonly present in the skin flora feeds on the oil secreted by sebaceous glands and in some individuals may be responsible for initiating or perpetuating inflammation as it feeds on the oil that is produced.
An Oral antifungal rather than oral antibiotics, specially Itraconazole would help in seborrheic dermatitis and itchy bumps over the scalp by decreasing the resident flora(Malasezzia furfur) which is implicated as one of the factors for causing scalp seborrheic dermatitis and itchy bumps. It is only available on prescription, therefore, I would advice you to take an appointment with a dermatologist in your region, before trying it
In my patients I also suggest them to use a 2% ketoconazole shampoo once daily or every alternate day as well. Ketoconazole is antiinflammatory antifungal as well as antiseborrheic. Lather it well into the scalp skin and leave it for 5 min before you rinse.
