Brief Answer:
Could be an external pile

Detailed Answer:
Hi
Thanks for writing in

It seems you have an external pile which is medically known as a sentineal tag and usually occurs when the fissure goes chronic

There are chances you are constipated and the external pile might have got thrombosed

Another possibility is a small perianal abcess which has ruptures spontaneously

Take sitz bath twice a day

Add a good laxative and for pain use diclofenac 50 mg twice a day

Also if possible send me the pics of the swelling to come to a conclusive diagnosis

Hope this helps
