What Do Fluid Filled Bumps On Penis Indicate?

Yesterday morning while having bath i noticed 2 tiny bumps on the skin of my penis. I pressed them lightly and little clear fluid came out of it. I straight away went to my std doctor to get diagnosed. My doctor said that it isnt anything to worry about. It is just a bacterial infection or due to friction from having frequent sex with my wife. And this time it was a bit rough too !! I am scared for syphylis and herpes as i had an uprotexted oral ( she performing on me. ) and all protected sex. 4-5 weeks ago with an escort who when asked said she was clean and ddnt have any std 's.
After this my doc still siggested that its nothing to worry. But i went and did the blood test any way for syphylis and herpes. My results have came back negative. For all rpr. Tcr. Tpr. And also hsv2 igG and igM are negative. Do i still need to worry about my results changing ?
Please help !! Very stressed !! As lot of literaute over the internet confuses me !

Hello and welcome to healthcaremagic
I am Dr. Kakkar. I have gone through your query and I have understood it.
I would be able to guide you better if you can upload a clear digital image of the genital lesions.
The duration between the appearance of the lesions and unprotected oral sex is roughly 4-5 weeks.
Herpes can be acquired through unprotected oral sex, however the incubation period is 3-5 days. However, initial infection may be clinically silent and may manifest as lesions later on. IgM antibodies to HSV usually appear within a week of infection. Negative IgM and IgG tests for HSV 2 rules out recent and past HSV 2 Infection but it does'nt rule out HSV Type 1 infection. I would have been more satisfied if you would have tested for HSV Type 1 too, because HSV Type 1 Virus commonly causes oral herpes and it can very well cause genital herpes if acquired through receptive oral sex from someone who had oro-labial herpes due to HSV Type 1.
Syphilis presents with a well defined, non tender, ulcer with a clean floor (chancre). The base is indurated and usually there is regional inguinal lymphadenopathy.
Incubation period of syphilis varies from 9-90 days. Therefore you are still within the incubation period of syphilis and moreover VDRL/RPR becomes positive about a week after the appearance of chancre. Therefore, a negative rpr immediately after the appearance of chancre does'nt rule out syphilis with surety. A specific test for syphilis becomes positive early, as compared to rpr
Therefore, I would be able to guide you better if you can upload a clear digital image of the genital lesions, for a clinical diagnosis
What do you mean by tcr, tpr? Its not striking to my mind.

Herpes genitalis, Upload a better picture
Detailed Answer:
Hi.
Thanks for the Image. It is not very clear but I could make out that there are 2 erosions, side by side with a yellowish floor.
Can you upload a better picture(preferably 2-3 ) and with good clarity, sharpness over the area of concern?
I am afraid but that looks like herpes to me most likely due to type 1 HSV
Apart from antibacterial ointments, I would suggest an oral antiviral e.g acyclovir tablets 400 mg, thrice daily for a week

Genital herpes
Detailed Answer:
Hi.
The pictures are not very clear and out of focus, but I could still make out that it is genital herpes. It looks like there are grouped erosions. It does'nt look like syphilis. Yes, do get yourself physically examined by a dermatologist OR if you can take better quality pictures mail me on my email ID YYYY@YYYY

Herpes
Detailed Answer:
Hi.
Negative IgM and IgG tset for hsv 2 is conclusive. I wanted to rule out if hsv Type 1 virus is the cause of the sores? since you had unprotected oral sex therefore there is a possibility of getting hsv type 1 from oral to genital skin contact, since hsv type 1 mostly causes ora-labial herpea.
pcr is definitely more accurate.
I wish you send me clearer pictures, that would be more conclusive on my part.
Regards

Hello doc.
I also did a hiv 1-2 test + p24 antigen test ( elisa ) and it came negative.
My hsv1 igG and igM results came back also negative.
Also as stated earlier my hsv2 igG and igM was negative. Still do i need to worry on my pcr test ? Please advise. I am still so scared ! Or what could be other reason for this symtons. P
Herpes
Detailed Answer:
Hi.
Well, the tests certainly don't justify the clinical diagnosis of herpes zoster. I would say that, if the lesions are healing you don't need to do anything different apart from applying a topical antibacterial and oral antibiotics. You may do pcr, just for completion.
But frankly speaking the lesions did look like Herpes to me. I have relooked at the pictures. The lesions though blurred look like multiple, grouped erosions. I again request you for clear digital pictures on my email ID ( YYYY@YYYY ). Can you do that? Herpes is difficult to miss on clinical diagnosis.
Regards

Possibility that it is not herpes
Detailed Answer:
Hi.
I can understand your concern and dilemma over the issue. I must say that the doctor who has examined you physically is in a much better position to confirm the diagnosis because he has looked at the lesions closely. Moreover, if the lesions are already healing you don't need to do anything different. You should stop worrying. The tests are not conclusive of herpes, either.
Take care and stay healthy
