Question: I taken 300 milligram twice a day. It has worked well until nap yesterday late noon. Woke up with jerking and memory was not clear for an hour. It,was,been very stressful here with hurricane XXXXXXX I am hav been dehydrated. Also I may have forgotten morning pill but do not think so.. my neurologist Dr XXXXXXX has retired. I was referred to Dr Lorin XXXXXXX who needs my records from XXXXXXX until scheduling an appointment. I am ok this morning and do not want to go to hospital

It sounds as if you experienced a possible breakthrough episode of seizure. If you're neurologist has retired then, you are well advised to find another as soon as possible. I understand that things may be a bit unraveled right now immediately following XXXXXXX however, if you've not had an adjustment to your medication regimen or have not had a recent neurological examination (past 12 months) then, you should have one following this episode.

If you were dehydrated, hypoglycemic, or for some other reason were suffering from a cold, flu, etc. then, that could also be the reason you had a breakthrough event. Make sure you stay well hydrated at all times. This is crucial for all people on antiepileptic medications.

I would not recommend changing the dose of your medications at this time until you've been seen by a neurologist. One suggestion would be to also obtain what are called TROUGH serum levels of the vimpat so as to measure how far they are dropping just before you take the next dose. This will help guide any increases in dose or frequency of the medication by your next neurologist.

Being compliant and taking every dose of medication is very important. Again, following a hurricane is a very stressful and difficult situation so I wouldn't fault anyone for having forgotten a dose of their medication but you should try to institute a system whereby you are always reminded to take your medications and not miss as this can very well lead to breakthrough events.

There are pill containers that have alarms on that. Most cell phones have an alarm feature that can be set to go off continuously or in your case....every 12 hrs....or whatever your schedule happens to be.

I don't think it's absolutely critical to go to the hospital at this point but you should get an appointment with a neurologist just as soon as possible...with or without records for a checkup and possibly some bloodwork to make sure you are taking the best dose possible of medication.
