Question: I have large septated cystic lesion on my left kidney measuring 7.5x6.7x5 x58 cm
it is anechoic with posterior acoustic enhancement what is it

I can see your concern.
Large cysts in the kidneys vary from benign to malignant. It is difficult to tell by just ultrasonogram alone. It depends on the wall thickness, septations, contrast enhancements, presence of solid mass or calcifications.
You need CT scan of abdomen with contrast and CT guided FNAC, which will finally tell what type of lesion it is.
If you can post the entire report of the scan, it will help us to grade the cyst so that, we can guess the possibilities.
