Hi,
Thanks for the reply.
I have not done a home pregnancy test.I didn't have intercourse for past 4 months and I have got periods after that.
What kind of tests in hormonal profiling you suggest? Could you help me with the list I have to check.
Yes usually I get soreness in breast but this time its there for longer duration.
How long after the breast soreness should I expect periods?

Brief Answer:
Tests advised
Detailed Answer:
Breast soreness usually precedes periods by a week or so.
In hormones you should get TSH, prolactin levels and fasting insulin levels.
Get these tests done and then update me the reports.
Also start some physical activity on daily basis in the form of gym or walk daily.
Avoid fast food and binge eating.
Take antioxidants in the form of green tea, salads and nuts especially almonds and walnuts and you will see a good difference.
All the best.

Hi,
Thanks for the info.
I will get the tests done tomorrow,
Is there any time when the tests have to be done ? i mean is it after the periods start or just anytime like tomorrow?
The 2nd thing is i am having white discharge with a tinge of light yellow for past 2 weeks or so.
Is it normal to get that?
How does the discharge change during periods? Does it become more clear like water?
Thanks for the info.

Brief Answer:
No relation with periods
Detailed Answer:
No relation with periods as these tests can be done anytime but preferred in morning and empty stomach.
Usually vaginal discharge thickens in second half of periodic cycles and especially a week before periods.
Watery discharge is common in first half of cycle just after periods are over.
Discharge is this common and nothing to worry if it is not foul smelling and there is no itching.

Left ovary: 3.8x3.5cms

No evidence of INTRA/EXTrA UTERINE GESTATIONAL SAC IN THE PRESENT SCAN
Thickened endometrium
Bilateral polycystic ovaries
Minimal Free Fluid in the pouch of douglas-Likely pelvic Inflammatory disease.
------------------------------------

I didnt have PCOS when i had a scan in XXXXXXX
Can it reappear because of stress. What are the other reasons?
What does this endometrial thickness mean ?
What is the fluid in the pouch of douglas? Is it critical?
THanks

Brief Answer:
All due to missed periods
Detailed Answer:
Hello mam, sorry for the delay in response.
PCOD is a clinical diagnosis and the ultrasound picture has a lot of observer bias.
It is quite likely that it was missed in your January ultrasound but that is not something to worry about.
This elevated endometrial thickness is because of prolonged missed period interval and will be fine after periods.
Iny opinion you should take progesterone withdrawal of periods by taking tablet norethisterone 5 mg three times a day for 3 days and on leaving the same you will get periods in 3-5 days.
And in next cycle take oral contraceptive pills after discussion with your gynecologist and continue them for at least 3 cycles.
On stopping them you will see regularity in your periods.
Fluid in pouch of XXXXXXX is seen just after ovulation and in some cases with pelvic inflammation.
That is nothing to worry as it will be fine after periods.
Right now just take progesterone as advised and you will feel much better.
Take care

Brief Answer:
Change your regimen
Detailed Answer:
Nothing to worry as its effects wane off after periods and thus no effects in next cycle.
But it is late for the withdrawal of periods as periods come within 3-7 days of stopping the drug.
So instead of taking these for withdrawal take same medicine twice a day from 8 September till 19 September.
And on stopping them you will get periods in 3-7 days and hence you will be free in that period.

Thanks for you reply.
An year back i had taken meprate for the same reason. That time I gt periods on 14th day after stopping so was confused.
Will it change with cycles for a person ? I mean last time if periods came on day 14 after stopping meprate, this time can it come earlier ?
THere is one more general ques for my understanding.
My friend was prescriped Duphaston 10mg for similar issue. How are meprate and this different ?
I am not planning a baby for next 1 year . I am not over weight.
Can this PCOS be managed and are there chances to get pregnant without complications ?
Thanks

Brief Answer:
Nothing to worry
Detailed Answer:
Hi XXXXXXX I know all your worries.
Actually PCOD alters the period pattern after stopping meparate and increases the gap between stopping the drug and onset of periods.
So there are chances that your periods commence very late after stopping the drug but since you don't want to take chance so better take it till 19 sept.
Duphaston is safe in pregnancy and thus is given to support pregnancy and in case there is no pregnancy then on stopping the drug it will cause withdrawal of periods as after meparate intake.
Don't worry about conception issues as not all PCOD patients encounter conception problem.
Just relax and take the life as it comes.
Take care

Brief Answer:
Not sure
Detailed Answer:
Yes dear take it as advised.
See the problem with PCOD is the periodic cycle cannot be predicted at all but usually they come in 10-12 days after stopping the drug.
As you are not planning for conception as yet you can take oral contraceptive pills with ginnette 35 or krimson 35 which have cyproterone acetate and thus will regularize periods as well as help in management of PCOD too.
Discuss this with your gynecologist and you will definitely observe a positive change in period pattern and flow.
All the best.

Brief Answer:
TSH is raised
Detailed Answer:
Hello dear, good to talk to you again and hope you are doing well.
Yes you are right your TSH levels are raised and thus you need to start tablet thyroxine 75 mcg daily in morning, empty stomach and avoid eating anything for at least half an hour after this.
Rest tests are fine except for raised LH levels as compared to FSH levels and thus suggestive of your PCOD status.
There is nothing to worry in general but in my opinion you should take oral pills for at least 3 cycles to achieve a regularity in your cycles and also will help in regulating the hormone profile.
Take care

Thanks for your reply.
For how many days should I have the TSH medicine?
Is there any medication for raised LH?
Also can these profiles become normal after 3-4 cycles?
Why I am asking is that in my test before 8 months or so the levels were normal so am confused.
What food items should I have and avoid for thyroid?
Thanks

Brief Answer:
Weight reduction and active lifestyle
Detailed Answer:
Thyroid medication is taken like food, never missed and usually taken whole life with minor dose alterations and 6 monthly repeat testing in initial years and yearly thereafter.
No food items alter its levels but in general an active lifestyle helps a lot.
Join some gym or start a sport activity, you will feel the difference.
As far as LH levels are concerned they are related to body weight and only help is reduction of weight in case overweight.
Metformin is advised in case cycles get very irregular or conception is desired but first take oral pills and then we will see in case there is need to start metformin also.
All the best.

Thyroid hormone will cause weight loss
Detailed Answer:
Hi dear, take the thyroid hormone medicine as advised regularly and it will keep a check on your weight and cause excess fat shedding too.
Birth control pills are known to cause slight weight gain but that is restored to normal baseline as soon as you stop raking oral contraceptive pills.
So nothing to worry and just start the thyroid hormone pills.
Take care and all the best.

Brief Answer:
Not much of weight gain
Detailed Answer:
No dear, birth control pills hardly cause weight gain of more than 1-3 kgs and that too is seen in people who are already overweight.
Since you have normal height and weight this will not be a problem for you.
Neither oral pills nor thyroid hormone therapy have any relation with hair loss.
Rather some studies show improved hair growth after thyroid hormone replacement to normal levels.
Thyroid hormone deficiency has a significant role in causing infertility so it is very important to have a euthyroid status before planning for pregnancy.

Brief Answer:
No periods while on pills
Detailed Answer:
Yes definitely fertility issues can be easily overcome by just maintained levels of TSH.
And as you plan to start birth control these irregular periods will be treated too and you will jot get periods after starting pills till you stop after completion of the pill pack.

So you mean to say that if today is day 3 of my periods and I start taking the pill today then my periods will stop tomorrow and will not come for the next 21 days because I will be on pills during that time?

Yes your periods will stop in a day or two and most likely will not come till the pill pack will last and also will give you contraceptive benefits.
Just don't forget any pill and try to take pills at roughly same time.
