Brief Answer:
Dietary supplements.

Detailed Answer:
Hi,
Thanks for posting the query.
I have gone through your query.
vitamin d is in very lesser range.
Haemoglobin is just borderline less, no need of any supplements.

For vitamin D i suggest you to take TAB.Daily cal which contain both vitamin D and calcium. Along with good exposure to sunlight before 8.30 am or after 4.30 pm

Need not bother much about the B12 levels, You can get it to normal by just increasing the diet rich in Vit B12 like chicken and fish.

Any query do follow up.
Take care
