What Do My Cholesterol Levels Indicate?

Question: My 63 yr old father has anxiety and high blood pressure 160/86 , cholestrol 270
Triglyceride 168,
HDL Cholestrol 57.0
LDL Cholestrol 201
VLDL Cholestrol 33.6

Doctor has prescribed following medicines
1. Prothiaden 75 1 at night
2. Esofag-D 1 morning
3. Krislow 100 1 in morning
4. olmin-ch 20 1 in morning
5. Amtaas 5 mg 1 in night
6. Roxulip 10 1 in night (for cholestrol)

My father now has BP 135/82 after having these medicines.
But he has also developed giddiness, breathlessness and choking in throat.
Above mentioned 3 symptoms are new and I am concerned how/why these symptoms have developed.
Do you suggest any change in the medicine.

Hi, thanks for the concern in the HCM.

As you have mentioned above your father has three problems-
1. Hypertension
2. Dyslipidemia
3. Generalized anxiety disorder

I can also see that your doctor has correctly prescribed medications for him.
Now, as you have mentioned, he has got three new onset symptoms-
1. Giddiness
2. Breathlessness
3. Choking sensation

Now, among all the medications, Dosulepin (marketed as prothiaden) is a tricyclic antidepressant, which is well known particularly for choking sensation, breathlessness and giddiness. Such side effects are exacerbated with simultaneous ingestion of Chlorthalidone (present in Olmin CH).

Therefore, you should consult your treating doctor at the earliest for revision of current regimen. My understanding is using some alternative medication to Dosulepin shall be able to stop such symptoms. Some SSRI may be introduced. You may also consult a local psychiatrist for psychometric evaluation of your father and better treatment.

Another important issue is that Chlorthalidone may cause electrolyte imbalance. Therefore, serum electrolytes level should also be checked at the earliest to rule out drug induced dyselectrolytemia.

An urgent ECG should also be done to rule out any cardiac cause like ACS or any illness related to ischemic heart disease spectrum.

I think you now know what modifications and evaluation are needed.
