Question: Hi Doctor,
Hope you are doing well!

As per our conversation yesterday and as suggested by you, my wife underwent PFT, Xray , CBC and AEC tests and I have attached the reports for your reference. Though we're consulting doctor here in XXXXXXX but would also like you to take a look on the aforementioned reports for your opinion. Will be thankful to you for this.

Please let me know if you find any issue while accessing or opening the reports.

I can understand your concern.
I have gone through the reports you have attached.
Sorry to say but I am not able to see chest x ray report.
Can you please reply me the result of chest x ray.
Her PFT report is suggestive of small airway obstruction. Her PEF 25-75 is low. This favour small airway obstruction.
And treatment of small airway obstruction is inhaled bronchodilators and inhaled corticosteroid (ICS).
So please let me know which drugs she has prescribed?

Hello Dr. XXXXXXX

Thanks a ton for all your suggestions so far. They were really helpful !!

I have again attached the X-ray report for your reference. By the way, everything is normal in her X-ray report.

She has been prescribed below medications by the Doctor at XXXXXXX Asia hospital in XXXXXXX Also attached the prescription for your reference.

1. Tab Montair FX HS (Once daily at bed time for 5 Days)
2. Tab Sompraz-D (Once daily empty stomach)
3. Benz Pearls capsules (Thrice daily for 5 days)
4. Tab Zynase-D (2 times for 3 days) : This is prescribed for pain due to tonsils in her throat along with salt water gargling.

Doctor has mentioned that she is allergic but which kind of allergy she has he didn’t tell us.

Please let us know if the medication prescribed are good enough for the treatment of her long running coughing problem.

Also, per your analysis of the PFT report which is suggesting small airways obstruction, do we need to take any other medication or need to take any further action for this??

Kindly take a look on the above medications and provide your valuable suggestion.

I can understand your concern.
Sorry to say but I am still not able to see the chest x ray. This is mostly some technical error.
But no worries as it is normal.
Drugs are appropriate. You should continue with them.
Wait for 5 days. Avoid dust, pollens, smoke, pollution etc. Ask her to wear face mask.
If still not improving after 5 days of treatment then we should start oral long acting bronchodilator (acebrophyllin).
So at present, continue these drugs and wait for 5 days. Tell me her progress after 5 days.

Honestly speaking small airway obstruction is characteristic of asthma.
To rule out asthma, I need to ask few things.
1. Is she having these symptoms regularly?
2. Are these symptoms worsen in dusty environment?
3. Are these symptoms present in any of her family members?

Hi Dr,

Hope you are doing well !!

Here are the answers to your questions :
1. Is she having these symptoms regularly?
Ans: She faced coughing issues in past also but that simply got cured using normal cough syrups and medications. However these days none of the medications are helpful. She is facing dry coughs for few minutes continuously and randomly these days specially in evening and night.

2. Are these symptoms worsen in dusty environment?
Ans: Not in dusty environment but in smoky environment(due to crackers or cigarettes) she always faces headache problem.

3. Are these symptoms present in any of her family members?
Ans: None of her family members have these symptoms.

Her symptoms are not confirming of asthma.
But we can not rule out asthma by these answers.
She is having triggering factor like smoke exposure.
She is also having recurrent cough.
So we have to think about asthma for sure.

3. Is she having whistling sound (wheezing) during sleep or during coughing?

Please find below the answers to your questions:

1. Is she ever undergone IgE level?
Ans: Yes, She had underwent IgE test in 2013 and it was 40.76 IU/ml. I have also attached the scanned copy of the report for your reference.

2. Is she having breathing difficulty while having symptoms?
Ans : No

3. Is she having whistling sound (wheezing) during sleep or during coughing?
Ans: Yes while sleeping she sometimes makes whistling/wheezing sound BUT NOT DURING COUGHING.

No need to worry for asthma. Her IgE level and eosinophil count are normal. She is also not having breathing difficulty. So asthma is less likely in her case.
So better to give the drugs for five days. She will mostly improve with these drugs.
Do let me know if she is not improving after 5 days.
