What Do My Lab Test Reports Indicate?

Question: Dear Doctor,
This question is for my wife. She is 22 years old. Her weight is 57 kgs and height 5 feet 3 inches. We are married for 2 years and 5 months, initially we used contraception’s (condoms) but we are now trying to conceive since last four months.
Right from the beginning her periods are irregular even before marriage. After marriage her periods got a bit regular for a short span of time but it again went back to irregular ways. She consulted a Gynecologist on 10/02/2015 and she suggested not to gain weight and prescribed NORMOZ tablets for a month and she also checked her thyroid profile, which came as normal and through her trans- vaginal scan report (attached) she confirmed PCOD.
On 14/09/2015 (3rd day of her period) she met her Gynecologist and trans-vaginal scan (attached) was done, and she prescribed few tablets which are as follows- FERTOMID 50 p (daily twice for 5 days from 3rd day of her period), ENRIFOL 2mg (one tablet for ten days), EVASERVE daily one tablet for a month (All the tablets from 3rd day of the period).
After one week of meeting the doctor, she again performed scan to check the Ovulation and growth inside (while using the above mentioned tablets). The report is attached. With these medications she started getting her periods normally ie 28 day cycle.

On 09/12/2015 She met her doctor and she changed her tablets to: REJUN 50mg (Daily twice for 5 days), PROGYNOVA 2mg (daily one for ten days), BERBESTA (daily one for 30 days) all tablets from 2nd day of her period, after these medication she got her period after 30 days now. Doctor advised to take these tablets for 2 months and try for pregnancy and if it doesn’t work she advised her to go for HSG test.

We both are trying to have a child since last September 2015 but somehow she is not getting pregnant. My wife has been taking above mentioned changed medicines for one month now. We are not sure whether we are going in right direction hence we need advise what to do next to come out of this problem and have a child. All her reports have been attached, please go through them and advice what needs to be done to come out of this problem, my wife is very worried about her PCOD problem. I have attached all her reports in sequential order as per date. Do let me know if you need any more information.

I saw the attached reports and they seem fine.

Since she didn't get pregnant HSG is must. Please let her get HSG done soon.

Moreover you need to get semen analysis done.

Don't worry about PCOS. many women with PCOS conceive.

Let her lose 5 km weight if possible.

Hi,
Shall we wait for another month and even then if she doesn't conceive, then shall we go for HSG test? or its better to go for the test immediately?
Are there any side effects to HSG test? I have heard that its a painful procedure, is it right?
What about the medication she is already using? Is there any need to change it or shall we continue the medication (progynoya, rejun, berbesta) for another month? She has been told to take these medication from 2nd day of her period. But we wanted to confirm it from you, hence she hasn't taken medicine yet. today is 4th day of her period. Now can she start these medication? Please advise regarding this.

My semen analysis is as follows: date 05/10/2015
total sperm count: 25 million/ml
MOTILITY:
activity motile: 20%
sluggish: 50%
non motile: 30%
MORPHOLOGY:
normal forms:30%
abnormal forms: 70%
The gynecologist also prescribed Fertisure-M tablets for me (husband).
Recently I have discontinued it on your advise.

So please let me know regarding medication of my wife? Can she start it on 4th day of her period (ie today)? and when shall I go for sperm analysis & HSG test?

Go for HSG this cycle

Hi,

HSG is mildly painful and you can request it to be done under anesthesia. It is better to get it done now and then go ahead with further treatment.

Dont take any medicines this cycle for wife and let her do HSG on day 7.

Your semen analysis is normal.You don't need any treatment.
