What Do My Lab Test Reports Indicate?

default
Posted on Wed, 9 Mar 2016
Question: Since my wife was pregnant in 2012, endocrinologist always insisted that we should strive to keep TSH above 1. In past one year we have observed TSH consistently below 1 and only once in July (when we traveled to XXXXXXX and might be due to missed dosage, TSH was reported above 1).

currently I am traveling and we consulted physician/nurse practitioner here in US but as per them till its in range we should not worry (even below 1 is fine). I wanted to reach out to endocrinologist and get opinion as what TSH level my wife should maintain and in case we need to adjust dosage to bring it greater then 1 please let us know modified dosage. Current dosage is 112 mcg of thyronorm per day

Test Report (2016_Thyriod_Test_Oct15-Jan16.pdf) XXXXXXX 1, 2016 - TSH 0.748
Oct 31, 2015 - TSH - 0.533

Test Report (2015_061215_Anjum_Thyriod_Vitamin.pdf)
July 3, 2015 - TSH - 1.520

Test Report (2015_040415_Anjum_Physical_Exam_April 15.pdf)
April 7, 2015 - TSH - 0.78

Brief Answer:
No need to modify the dose

Detailed Answer:
Hi,

Welcome to HCM. I have gone through your reports.

Her TSH and free T4 is currently normal. The target TSH we usually keep is from 9.5 to 2.5 ( individual doctor opinions may vary). Her TSH of 0.78 is within this range. So my suggestion is to continue with the same dose of thyroxine ( Thyronorm).

If she is not comfortable with low TSH ( have palpitations, weight loss or heat intolerance) and prefer to keep TSH above 1, you can decrease the dose to 100 mcg and repeat the test after 6 weeks.

Hope I have answered your questions. If you have any further questions, I will be pleased to help you.

Regards
Dr Ajish TP
