I have seen all the reports in detail.
Your MRI report as you must have seen is not showing a good prognosis as far as treatment without surgery is concerned.
Main issue is of bilateral involvement and thus making surgery mandatory and pour future fertility.
In your blood reports everything is largely normal except vitamin D levels which are very low.
So my management plan for you is to get CA-125 levels done as pre work up for surgery and then visit a good laparoscopic surgeon and get the cysts removed at the earliest.
I would like to know if you have completed your family to yet to do the same?
In the meanwhile you need vitamin D supplements in the formidable weekly sachets of 60,000 units of vitamin D for 6 weeks ( but it is a prescription drug so visit your doctor for it).

Alright there is not much to worry now then and the back pain you mentioned is just because of low vitamin D levels indicating osteopenia and thus can be easily treated by vitamin D supplements along with physiotherapy.
Also I would advise you to get a proper pelvic examination by a good gynaecologist so as to rule out any pelvic infection (since your ultrasound has a free fluid in POD).
Pelvic inflammation can also cause weakness and backache problems in some patients.
