Hello,
you haven't mentioned your medical history and the drugs you take, so my answer will be based on your reports only. Since you've done the glucosylated hemoglobin (HbA1c) measurement I suppose you do know that you have diabetes. Your HbA1c proves that your diabetes is not under control though and measures have to be taken. I can't suggest anything but general measures for it, because I don't know what's your current treatment regimen.
Physical activity, healthy diet and control of body weight are the most essential interventions for diabetes. If you're overweight or obese, loosing some weight will help a lot to control your blood sugar. Walking every day for 20-30 minutes would be very beneficial for your health. Drugs might have to be added to your regimen but only after a careful review of your current regimen and the measures I've already suggested.
Your triglycerides are a little higher than the target levels but this is not too important taking into account that your LDL cholesterol is low.
Your hemoglobin is low and a good explanation is required for it. If you don't know the cause then investigation is recommended.
