What Do My Lab Test Reports Indicate?
Please refer to the picture attached. The area of the swelling is circled in red. The rectangle black box is the region which hurts a lot when pressed.
My doctor advised X ray and URIC acid test. X ray is normal and it is not a sprain. URIC acid was 7 but with controlled diet brought it down to 6 but there is no improvement. I have had naprosyn for 5 days but no use. What is this problem and what is the treatment.

Detailed Answer:
Hi xxxxx
Thanks for the query
I have read your query and viewed the attached photograph.
Usually Gout presents itself with pain and swelling in the joints of the foot. Blood Uric acid may even be normal in Gout but the main diagnostic test is to check the Uric acid level in the fluid aspirated from the affected joint.
The marked region in your photograph points to the 2nd metatarsal head. Sometimes there could be arthritis of the joint in this region or a stress fracture.
Keep the limb elevated. Apply ice to the affected region. Take pain killers as instructed. Avoid weight bearing if painful.
Naproxen is not a treatment for Gout. It's just an Antiinflammatory Medicine for the pain and swelling but it will work only within its limit. You may need to take Uric acid reducing drugs like Allopurinol or Febuxostat.
Do get an Xray, Serum Uric acid, Urine Uric acid and upload the results for my review.
I hope I have answered your query.
I will be available to answer your follow up queries.
Regards,
Dr.Aashish Raghu

Therapy
Detailed Answer:
You can wear silicone padding for metatarsal heads in your footwear. Also use Microcellular rubber slippers.
Perform arch stretching exercises and dip your feet in XXXXXXX warm water with added salt twice a day.
If this does not give you relief, you can get an ultrasound of the painful region to look for joint capsule swelling and get a joint aspiration with needle to test the fluid for Uric acid. Also steroid injection can help with the pain.
