Mental state and wellbeing at assessment
Miss C presented in her electric wheelchair and said that her friend accompanying would be in the session also-a fellow wheelchair user. She was well kempt and had clearly paid attention to her appearance in a black and white dress.
She reported as she talked about her medical history that she waes left with problems with understanding and memory, and in attepting to obtain detail of historical sequences, it was clear that there was to some degree a difficulty in this regard, although as regards understanding, Miss C seemed very well versed and quick to pick up issues. Her history was both detailed and nuanced as she unfolded it, perhaps illustrating an affect of her having been in years of counselling and having developed a narrative.
Miss C was visibly upset and moved when mentioning the reasons she constructed about her adoption away from her birth family and her descriptions of various people her background was infused with insight and warmth.
In summary it seemed to me that she was doing exceptionally well in her circumstances, and while her self harm ideation was of concern, she was aware of it and with her counselling attempting to get underneath her difficulties.
Formerly at interview, Miss C's mood was euthymic and reactive, speed was spontaneous and coherent. There was no evidence of hallucinatory or delusional material; of passivity phenomena or thought disorder.
Miss is a 33 year old Spina bifida wheeelchair user referred to the PDCS at the instigation of her counsellor following concerns about her self harm thoughts. While these sound dramatic and illustrate internal frustration that she describes, they would not seem to reach criteria of risk sufficient for her to be taken on by secondary care. .
I would very much appreciate someone interpretation of this report and to explanation of their understanding of what the report is telling me, as soon as is possible. Many thanks
