I carefully reviewed your uploaded ECG recording and would like to explain that except for a slightly prolonged QTc interval, there isn't any dangerous abnormal finding.
As you are have several coronary risk factors (hypertension, diabetes, and likely hyperlipidemia), I would recommend an exercise cardiac stress test for ruling in/out a silent cardiac ischemia.
If any abnormal finding results, then further clinical investigation is necessary (coronary angio CT, etc.).
But, first of all, it is strongly recommended optimizing your diabetes therapy (as HbA1c 12.5% is quite high). Probably you need to start insulin.
You need to closely monitor your BP values for a couple of days up to a week (several times daily) and review those values to decide the ongoing anti-hypertensive therapy.
Also, I recommend checking a fasting blood lipid profile as an antilipemiant therapy is almost always necessary in the settings of diabetes.
I recommend discussing with your attending physician on the above mentioned issues.

My Haemoglobin count is 12.5% and not the HbA1c , Hb1Ac is 6.0.
My Physician has asked me to meet him after two months.
Please suggest actual picture of the Report and what needs to be done by me to be healthy without any complications.

I am sorry for the misunderstanding. I thought HbA1C was 12.5%, 6% is excellent.
So you do not have to worry about uncontrolled diabetes, because your blood fasting glucose seems to be within the normal ranges.
Hb levels of 12.5mg/dl have excluded possible anemia.
Coming to this point, I would recommend monitoring your blood lipid profile. In case of increased cholesterol levels, a statin should be considered.
Meanwhile, I would recommend following a balanced diet (Mediterranean diet is recommendable). Increasing omega 3 fatty acids in your diet (fish, fish oil, walnuts, olive oil, etc.) would help maintain a normal blood lipid profile.
I would also advise performing a lot of physical activity (walking, aerobics, jogging, recreational sports, etc.), which is necessary to reduce your cardio-vascular risk.
