I have read your query and understand your concerns.
Endometrial thickness of 13 mm is normal and it varies accordingly to your periods. Fluid in pouch of XXXXXXX can be due to pelvic inflammatory disease. May I ask for what reason you underwent sonogram. What were your complaints? I will be happy to hear from you.

Brief Answer:
Ultrasound report needed
Detailed Answer:
The thickness of endometrium varies from 4-15 mm according to the day of the cycle especially in reproductive age group like yours.
So thickness is not a problem at all.
But in case you are having pain then that has to be addressed.
The size of the fibroid is the main thing and also I would like to know if you are having normal periodic cycles and glow in periods.
also if the pain is only in periods then it can be easily managed but if you have continuous pain irrespective of the cycles then you need antibiotic treatment as there is fluid in POD too.
Can you send me your ultrasound report so that I can advise you even better.
Regards
