What Do My Test Reports Indicate?

Question: Very happy New Year for all of you esteemed doctor team and healthcaremagic.com team as well, sorry for this long qwery....but couldn’t stop myself in explaining my condition in detail.

A.
I am a female 30yr/ weight 72kg/ height: 5’.4” married 9 months ago in April 14, my periods has always been delayed for 45-55 days since its inception.
1. We Visited a doctor prior to planning for a baby (preconception counselling); their I was asked to undergo various blood tests like CBC, TSH, RBS, S.PROLACTIN, USG UTERUS AND ADNEXA, later came to know I have been suffering from PCOS.
2. ULTRASOUND STUDY OF UTERUS & ADNEXA FINDINGS:
UTERUS: measures 8.6X3.7X4.74cm
BOTH OVARIES: Bilateral ovaries are enlarged in size. Multiple small follicles are seen with thickened echogenic central stroma.
Right Ovary Size : 3.7X2.0X2.4CM (~10ML)
Left Ovary Size : 3.5X2.2X2.2cm (~9ml)
Impression: Bilateral polycystic appearing ovaries.
3. USG FOR FOLLICULAR STUDY done on 12th, 15th and 20th day with finding endometrial thickness grow from 7mm to 9mm right ovary largest follicle size grows from 14x11mm to 15x12mm left ovary largest follicle size remains same 7mm.
4. Some of her prominent report findings:
i. PROGESTERONE SERUM           : 0.48
ii. GLUCOSE FASTING           : 82
iii. INSULIN FASTING           : 4.8
iv. FSH      : 5.12
v. LH      : 8.52
vi. PROLACTIN SERUM           : 6.42
vii. TOTAL TESTOSTERONE           : 62.60
viii. FREE TESTERONE      : 1.72
ix. LIPID PROFILE      : NORMAL
x. SERUM CREATININE           : 0.57 (LOWER)
xi. CBC      :NORMAL /except eosinophils (8.4)&ESR (15) raised
xii. BLOOD GROUP           : B positive
xiii. TSH      : 0.784


5. Period checker;

6. Current Medication: (started on 25th September 2014) for 5months (ending by 25th February2015) along with daily 30min-1hr exercise for her weight loss.
i. GLYCOMET SR 1-0-1
ii. MCBM 69 0-0-1 (Folic acid)
iii. CRISANTH 0-0-1 (Started on 28th September i.e, 5th day of period).



In August 2015, while pre-conception medical check up I have been diagnosed with PCOS and was put on medicine:
1.     Glycomet SR 500 1x2
2.     MCMB 69 1X1
3.     Crysanth (21 tab every month starting on every 5th day of my period)
Now its been 3 months taking the medicines my periods are on time, we are panning for baby by Feb 2015 i.e, 2 months from now we have been advised to continue the medicines for another 2 months.
Qwery:
1.     Is it safe to take the medicine for another 2 months (altogether it would be 5 months in a go).
2.     How serious is this PCOS condition and can it be hurdle in conceiving naturally?
3.     Say in Jan/Feb 15 we plan for baby naturally and it fails, what would be the next step?
4.     OR safer shall we go for IVF or any alternative way which may increase chance of twins as well (like drug- clomiphene citrate -given for Ovulation Induction may have chances of twins).
5.     My own cousin married 4 yrs back and his son 4yrs. diagnosed with ADHD and moderate Autism (though none of us in our family history have these illness); precautionary do we have to induce any drug as a preconception medication to prevent this kind of illness.
Kindly suggest further action we should take after completion of course of drug she is taking or require to induce further any drug for assisting conception.

Note: Headache & feeling like lightning across eyes sometimes, appetite is very less (doesn’t feel hungry), since long 4 roti (Indian bread) and a cup of rice and little vegetable a day.

1) It is safe to take medicine for another 2 months.

2) PCOS is not serious condition and you can conceive naturally.

3) If you fail in XXXXXXX / Feb, go for IUI ( intrauterine insemination) or IVF procedure.

4) IVF is best method if you are willing. Go for it.

5) You dont need to take any precautions for ADHD.

6) The diet you are taking is adequate for you.

Dear Doctor,
Thanks for earliest reply.
Few follow-up quick-qweries:
1.     Glycomet SR 500mg, taking it could lower my blood sugar level too, is it necessary to check my blood sugar frequently?
2.     Sometimes I skip breakfast or dinner but I am not skipping Glycomet pills, shall I continue or stop during those missing meals (may be it can lower my blood sugar).
3.     My fasting blood sugar is 89 and random is 100.
4.     Along with it I have been diagnosed with my early stage of depression and psychological treatment like cognitive behavior therapy to correct your perceptions about self and others and advised to take escitalopram(antidepressant) and ativan (for sleep) is it safe taking this pill alongwith existing medication.
5.     Also is it safe conceiving during this medication and the therapy advised state of mind.
6.     If I postponed our family planning which we thought by Feb 2015 till this therapy don’t know how long it go, how long shall I continue the above mentioned PCOS related medicine Glycomet-MCBM-Crisanth.
7.     I stays in Banglore, please get me your contact details to be in touch with you also looking for a Psychiatrist at XXXXXXX for undergoing psychological treatment like cognitive behavior therapy or any alternative treatment like any Holistic center at XXXXXXX
8. I have been asked to undergo test Insulin F & Insulin PP (how is these test are different from normal blood sugar test-fasting-pp-random).

1. Dont worry about lowering of blood sugars to an alarming extent. You dont need to check blood sugars frequently.

2. Anti depressants can be continued. They dint interfere with metformin.
