What Do My Ultrasound Test Results Indicate?

Question: Hi,

I am continuously having Pain in Right side of Abdomen, somewhere in Kidney. I got it scanned but Report I got is bit confusing. Kindly Explain this to me that What Problem I currently have ??

I am a Diabetic, with un controlled diabetes, which ranges between 250- 370 &
I am taking below medicines for diabetes i.I.
1). Azulix 1 MF forte - Morning Evening
2). Cyra LS - Morning only
3). Cap Optisulin - Morning Only.
I never miss my medicine nor I eat sweets Still my Diabetes is un controllable, If Need to change medicines with Insulin or what you Suggest me ??

Thanks for your question.
Noted your concern.

In such case I may suggest immediate medical attention and further evaluation. Uncontrolled blood sugar with such Ultrasound finding may lead to pus formation in kidney (Pyonephrosis or Pyelonephritis).


You should consult near by Diabetologist and Nephrologist immediately without further delay.

I may suggest-
Detailed clinical examination depending upon it following investigations may be suggested.
- Complete blood count.
- HbA1c.
- Urine examination including urine culture & sensitivity.
- Liver Function Test.
- Kidney Function test.
- CT scan KUB (Kidney, Ureter, Bladder)

For blood sugar control I may suggest Insulin with frequent monitoring of blood sugar level.

Cyra LS is antacid with prokinetic which may be continued.
Optisulin is a micro-nutrient supplement .
Both of them have no direct effect on blood sugar control or kidney issue.

Brief Answer:
Changes of obstruction.

Detailed Answer:
In your report there are changes in right kidney due to obstruction of ureter though cause of obstruction is not clear.
PCS (Pelvicalacyal system) it is a draining system which collect urine produced by kidney and drain it in to ureter (connecting tube from PCS to urinary bladder).
Dilated PCS means PCS enlarged due to back pressure due to obstruction.
Same changes in upper part of ureter (dilated upper ureter).
These changes denotes possible obstruction in mid part of ureter.
Due to stagnant urine possibility of infection increases which is more with uncontrolled blood sugar level.

Accepting your advice I have taken appointment from a leading Diabetologist of Fortis Hospital.

My Follow up concern is :
1). Dont you think that this obstruction can merely be the result of Small Kidney Stone, which have traveled to ureter ??

2). My Liver is also showing fatty, If its also due to diabetes or its different problem... I have gas problems & I have learned that it might be due to fatty Liver..

3). If I have to start Insulin at my own what should be the qnty it should be ??

Relieving obstruction is more important.

Detailed Answer:
Yes calculus may be the cause for obstruction. But whatever the cause relieving obstruction is important.

Fatty liver may be due to insulin resistance (NASH – Non Alcoholoic Steatohepatitis) it need to be correlated with Liver function test.
Gas problem may be a part of fatty liver or may be due to diabetic gastroparesis.

Depending upon Liver and Kidney function test appropriate decision regarding continuation of oral anti-diabetic medicines and starting insulin can be taken.
Insulin doses need to be adjusted according to frequent monitoring of blood sugar level.
For rapid control of blood sugar level insulin is preferred.
