Question: My mother age is 56 and she has diabetes problem , her sugar level is always 190 above , she was complaing about strech and kind of gas in abdomen area but when went for some blood test she has hpylori postive which random glucose level 280 with fatty liver and after doing ultra sound report said she has little swelling on her left kidney with 0.5cm kindney stone . What go be done for stone and high glucose level and fatty liver.

Thanks for the query

I understand your concern about swelling of the kidney secondary to a kidney stone. The treatment of the stone depends on where it is present, so if you could scan or type out the report I can help you in a better way. However, 0.5mm stone is a very small stone which most often does not require treatment, drinking plenty of water ensures the stone to pass away on its own.
Now coming to the bloating of the stomach, H Pylori is often implicated with such symptoms. It is easily treatable. It has to be treated with Anti-H pylori measures. It popularly comes a HP-Kit or Pantop HP kit, which is combination of three tablets to be given for 14 days. In addition to that I recommend that following changes in the diet

1. Avoid milk and milk products
2. Smaller meals several times a day is better than three large meals.
3. Do not sleep immediately after dinner/lunch. It is important to stay upright for at least 2 hours
4. Walking for 30 min a day will help reduce bloating significantly
5. It is important to have plenty of fruits every day. Having salads might worsen the bloating in a few patients

Thanks for getting back. I am sorry for the short delay in my reply, I was away for a conference

The renal stone is very small and does not require treatment at the moment. I recommend taking 3-4 liters of water a day and rechecking for renal
stones after a week, if the stone persists or if the pain is increasing then she needs to undergo lithotripsy. ( Simple, painless day care procedure ).

The sugar levels are high and needs adjustments. Here is what you should do

1. check fasting blood sugars and post prandial blood sugars
2. let me know the medications she taking currently for blood sugar
3. tell me her weight and height

Hi.

Yesterday i went for few more tests for her, fatsing augar came 152 and urine had very nominal infection and serum result was fine. Her height is 5.6 and weight is 82 and she takes galvus met 1000 and amryl 3.0 , she takeboth medi twice a day.

Here is what you must do

1. Urine infection can cause uncontrolled sugars. It is mandatory to treat even a nominal Urinary tract infection with antidiabetics in a diabetic patient
2. She is slightly overweight for her height. I recommend losing at least 7 kilos. This will help further reduce fasting sugars
3. It is mandatory to walk daily for 45 min a day
4. If I were to be your doctor I would add Pioglitazone once the infection is treated. It is taken as 15 mg once in the evening. Please speak to your doctor regarding this

Fatty liver is a benign condition most often because of increased fat around the liver. It is often associated with abdominal obesity. The only treatment advocated would be to do abdominal exercises. Please speak to your gastroenterologist regarding this.
