What Do My Ultrasound Test Results Indicate?

Question: My Total PSA in September 2015 was 1.10 ng/ml and Free PSA 0.38. I am currently under treatment for the past 6 years for early IGA with creatinine of 1.2 mg/dl. and proteinurua In July 2016 my Free PSA was 0.39 (I did not get my Total PSA test). I took an ultrasound and the diagnosis was that kidneys were normal, prostrate was 4.4 x4.1 x 4.2 cm with volume 39.99 cc. Post void retention was 2 ml. Impression was Grade 2 prostratomegaly benign. All other organs normal.
I do not have symptoms of night urination or too frequent but I think the urine flow is not strong.
What is the recommended course of action I should take i.e. is it advisable to start medication? Should I also get my total PSA tested? Should I repeat ultrasound and PS tests every 6 months? Thanks

Thanks for your query ,based on the facts that you have posted it appears that you have obstructive voiding symptoms like urine flow due to Benign Enlargement of Prostate which is common after the age of 55 years .

Since your ultrasound scanning revealed Prostate size to be 39 cc with post void urine residue of only 2 ml, the poor urine flow that you have can be improved by taking combination of Tomsulosin and Dutasteride once daily.

Please get your Uroflometry study done before you start medication to know the average urine flow per second this will help us as a baseline to judge the improvement in urine flow after taking medications.

As regards your PSA level it is absolutely fine and does not require any active intervention .

You may repeat PSA and USG every levels every six months .
