What Do My Urinalysis Test Results Indicate?

default
Posted on Sat, 7 May 2016
Question: Hi -- My daughter has been suffering for about a month with a feeling of urgency…absolutely no pain or burning, either in passing urine or when being physically examined. Her urinalysis shows moderate squamous epithelial cells, moderate nitrates, and a value of 50,000-100,00 CFU/ML lacto strep. Everything else appears to be normal. She has been checked for diabetes and has also had both bladder, kidney and pelvic ultrasounds all of which appear to be normal. She has been on three different antibiotics. Five days on bactrim, ten days of flagyl and now she is finishing a five day course of macrobid. She seems to get relief from the antibiotics but at least in the first two cases about a week afterwards, urgency is back. She is due to see a urologist in a week or so, but everyone seems to be puzzled. She is 19, healthy, but has had a major spinal fusion several years ago. Her pediatrician implied condition might be nerve related, but why would the antibiotics relieve the urgency if it involved nerves? Any thoughts?

I am Dr Saad Sultan and will be answering your query. I have read and understood your health concerns.
As far your case is concerned, if she is alright after spinal fusion and developed urgency now , then I don't think it is related to nerve problem.
On the other hand , if urgency was there and now it has increased then it may be due to superadded infection.
Urinalysis which you have described , definitely pointed towards the infection.
In my opinion , she really needs culture and sensitivity testing of urine with antibiotics being off for at least 48 hours. Apart from it, ultrasound scan of post voidal urine volume should be noted.
She may needs antibiotics , drugs for urgency and bladder exercises to cope with it
