Looks to be be neurological

Detailed Answer:
Hi.
Thanks for your query.
Noted the whole history you have provided and understood your concerns.
Since you have multiple problems and are on many medications as noted by you, the most probable cause of the problems of waking up with serious drawing and contractures of ankle and toes, left thumb symptoms, light headed and poor balance may be related to the already existing problems and/or added-on by the following possible reasons.
You are a known patient of neuropathy hence the most probable reason.
Light headed and poor balance might have a manifestation of some brain problem for which you need the following:
Clinical evaluation, examination by your Doctor, preferably by a Neurologist and second opinion of an ENT to rule out any ENT problems.
Relevant investigations of blood, Nerve conduction studies, MRI of the brain, mastoids and cervical spine as indicated by clinical examination.

These will give the best possible diagnosis and a plan of further management.
I hope this answer helps you.
Please feel free to ask for further relevant queries if you feel that there is a gap of communication.

Brief Answer:
Thanks for appreciation.

Detailed Answer:
ENT means Ear, Nose and Throat Specialist.
Also get the blood tests for Serum Electrolytes, Calcium, Vitamin D, Thyroid functions and others as indicated. There are chances that some abnormality may be detected and proper treatment gives dramatic results.
For example you have mentioned about carpal tunnel syndrome and low thyroid is one of the reasons which if found and corrected shows dramatic effects.
Please give feedback and reports of tests once done.
Wishing you all the best.
Warm regards.
