What Do Persistent Weight Gain While Dieting And Joints Pain Indicate?
Question: I have a few different labs that are unusual- HemoA1C - 6.0, C-Peptide- .76
Ultrasound showed normal pancreas but enlarged liver. Further blood work showed elevated GGT (47), elevated CPK (280)
TSH while still in the normal range is higher than previous years (2.56 today, 1.51 last year), T4 is normal- 1.14
Everything else in metabolic panel is in normal range. Any thoughts on the elevated liver enzymes and what type of a specialist would I see to discuss?
I'm an athlete, 5'8", 135 lbs, 58 years old. Concerned about muscle breakdown, recent weight gain even while dieting, and achy/sore joints all the time.
Any advice is appreciated, my PCP has suggested rest and retest in 3 months but I'd rather do something sooner if this is a problem. Thanks.

From the above reported exams ALP is high (if the XXXXXXX of normal range is 145) and GGT very slightly elevated.
Having into consideration your age i would suggest to check the bone biochemical panel (calcium level, phosphates , magnesium, Vit D, parathormon) to rule out osteoporosis as a cause of high ALP.
HbA1c level is slightly elevated and the fasting blood sugar and postprandial ( or a OGTT test) should be checked.
The lipid profile should also monitored closely as the liver is enlarged ( probably fatty).

Thank you for your input. Would it be unusual to have fatty liver disease when I am not overweight? I am very active- and I wonder if my liver has become glycogen depleted too often- would that be a cause of inflammation?
I will ask my doctor about the tests you suggested, but I did recently have a bone scan done with no signs of osteoporosis.
I suspect muscle damage is the cause of the elevated enzymes, but I don't know what that means long term. Just rest? Is that my best approach? I love to run, but it seems that this may be part of the problem. Thank you again.

If the cholesterol level is high it may be a fatty disease even if you are not overweight. Any way the tests for hepatitis should be done.
The level of fasting and postprandial glucose will give more information regarding to the cause of high HBA1c. If you do not have family members with Diabetes and the weight you have gained lately is unjustified than i would to check the cortisol level.
If the bone scan is normal than you may check only Vit D level. It may be one of the causes of weight gain.
Please accept my apologise regarding to CPK, for a while i confounded it with ALP.
In my knowledge, there are different ranges for CPK in athlete. The laboratory has shown that CPK is higher in athletes than in nonâathletes, whether they are male or female, and the normal ranges are different for athletes.

So, a physician that deals with athletes would give you a proper answer rather this level of CPK is considered high for an athlete.
