What Do Recurrent Episodes Of Palpitations And Sweating Indicate?
I am a woman 64 years old, retired, in good health, went through menopause without any issues ten years ago, I have never smoked and only drink occasionally moderately. I eat a balanced healthy diet, exercice regularly ( swim,bike, yoga , walk) and enjoy a good weight , sleep and digestion.
However I have in the last few years experienced episodes sudden and short moments of heart racing combined with feeling hot and sweaty, this also inevitably occurs if there is loud noises around me.
I also have over the last years lower levels of energy as I get quickly tired, and sometimes after a busy period (like Xmas) feel exhausted for days .
I then rest and then I recover and get on with an active ordinary life.
Now during the last few week I have felt more intense fatigue in the evening particularly , but yesterday after hosting a family lunch , enjoyable but particularly noisy with young children , I was left feeling shaky, heart beating hard and exhausted, and now over 24 hours later I still feel completely drained ... I don't think this is normal ...
I am not on any medication .
I would love your opinion on this.

Dear XXXX,
I hope I have spelt your name correctly, if not, you may correct me.
At the outset, I must complement you for successful and comfortable management of your menopause for last 10 years.
At this age, you are vulnerable to following disturbances:
1. Due to menopause, there is Oestrogen deficiency which does cause hot flushes, mood changes.
2. You have to rule out the possibility of Diabetes as these symptoms may show up due to raised or lowered blood sugar.
3. The possibility of Thyroid dysfunction at this stage of life is quite high. Thyroid dysfunction does cause mood swings, hot flushes and palpitations.
4. High blood pressure has to be kept in mind.
Irritation by noise is a classic feature of high blood pressure, Thyroid dysfunction and uncontrolled diabetes.
I would advise you to :
1. Get your physical examination done including Blood Pressure check up from a physician. Cardiogram would be very necessary and informative at this juncture.
2. Get your fasting and post-prandial blood sugar level examined. Also HbA1 would be more informative. This would help us to look for the diabetes.
3. Please get your blood examined for Thyroid function test (T3, T4 and TSH) to assess your Thyroid function.

Mean while, please continue your exercises, yoga and meditation. Addition of Phyo-oestrogen food to your diet will give you some relaxation. Soya, broccoli, are few items having phyto-oestrogens.
Please do not start any Hormone Replacement Therapy unless all these reports are viewed and studied by an expert.
