Question: Hi,

Following up after the following thread
http://www.healthcaremagic.com/AskDoctorInboxServlet?page=viewQuery&queryId=195778

My mother has had 6 doses of Taxol till now and the side effects have been bearable till now. Slight loss of appetite on day 2 and day 3 after the Taxol, and other than that, she is doing okay.
However, the blood count has been on the decrease steadily one chemo after another. I have attached the reports just before her 6th Taxol and before her 1st Taxol.
We were wondering if she could take a two week gap instead of one week before her 7th Taxol

Brief Answer:
No need to give gap

Detailed Answer:
Hi
Thanks for followup.

It is good to know that she is doing well. Though the total leucocytes have reduced, the neutrophil counts are well above permissible limits to continue chemo. We may give one dose of GCSF before the next blood test so that the count improves again.

I don't think there is a need to give a gap.

Hope this helps.

Hi,

Will giving a gap do any harm? Are there any studies which suggest that.
Will GCSF help with lymphos?
Normally, in your experience, how much do the blood counts reduce over the entire duration of 12 weeks?

Brief Answer:
GAp should be avoided unless indicated

Detailed Answer:
Gaps can do harm. But obviously for one week gap in the total 12 weeks will have minimal impact, if any.
But still, unless absolutely indicated, we do not advocate gaps.

GCSF only boosts the neutrophils, not lymphos.

The blood count reduction is extremely variable between patients. Some patients do not require any GCSF at all, while some require it every week.

Is it okay to take GCSF just before chemo? I sn guessing, If neutrophils are rapidly dividing during chemo, they may be more vulnerable to chemo cytotoxicity.

Brief Answer:
yes okay

Detailed Answer:
But for the reason you mentioned it should be at least 24 hrs prior to the chemo.

I meant to give her gcsf before the 'next' chemo, after the coming one.

Hi,
I have attached her blood report from today. TLC is 3200 and neutrophils is 67%. Can she get the chemotherapy done tomorrow morning?
