Brief Answer:
Get examined and investigated by a neurologist
Detailed Answer:
Hello,
I have gone through your question and understand your concern.
The mri is more suggestive of small vessels changes due to hypertension.
Though risk of multiple sclerosis is high in siblings but doesn't seems to be in your case.
The headache most likely is migraine like if it is associated with transient visual symptoms or smell problems.
You need to get some blood test like blood sugar, thyroid profile, vitamin B 12 and vitamin D.
Visit to a neurologist for detailed examination is required.
Hope you found the answer helpful.
Do get back to me for further information.
