MRI findings suggests degenerative disc disease of lumbar spine.
Detailed Answer:
Hello
MRI findings suggests degenerative disc disease of lumbar spine.
It is due to chronic stress on annulus fibrosis.
There are spondylotic changes and loss of water from intervertebral disc at L4-5 level.
There is also hypertrophy of ligamentum flavum at L4-5 and L5-S1 level.
At L4-5 level, there is diffuse disc bulge with herniation of disc in right central and para central region which is indenting the thecal sac anteriorly.
There are also few small disc bulge which is indenting the thecal sac.
There is no compression of nerve roots at any level.
Spinal canal diameter is also normal.
Majority of the cases of degenerative spine disease are benefited by physiotherapy and analgesics.Since you have mild findings,so you should try physiotherapy first.Surgery should be the last option.
Treatment can be planned after consultation with neurologist.
CT scan findings suggests mild fatty changes in liver(fatty liver) and mildly enlarged prostate.
Fatty liver may be due to many reasons like hepatitis, alcohol intake, obesity, altered lipid profile, medicines, auto immune causes etc.
Fatty liver is reversible condition.Obesity and sedentary life style are its common causes,so these factors should be ruled out.
You may need to change your diet and life style. You should a go for brisk walk regularly.
Mildly enlarged prostate is most likely age related finding. You may need to take medicines after evaluation.PSA(prostate specific antigen)should also be assessed.
You may also need few routine investigations like routine hemogram, random blood sugar, liver function test, lipid profile, urine RE/ME.
Weakness etc may be related to many causes like raised blood sugar, altered lipid profile, anemia etc.
You need proper clinical evaluation by a physician.
Findings are not serious. You will be fine.

I have attached the recently done health checkup report. Kindly review it and let us know what should be our next step.
Major problem is elevated blood sugar. It should be controlled.
Detailed Answer:
Hello
I have gone through your findings in detail.
Major problem is elevated blood sugar.Besides diet restriction, first I advise oral hypoglycemic drugs like metformin, glimepiride etc to my patient.
Vitamin D is also decreased. You may need vitamin D supplementation. You may need to take vitamin D tablet weekly once for eight weeks then one tablet can be taken monthly for six months.
You should also take calcium rich foods and expose yourself adequately to sunlight at least for 5 mins daily.
Serum lipase is also raised. It is specific for pancreatitis, but your CT report doesn't indicate this.It need clinical correlation.
High Sensitivity CRP is also raised,so it is important to evaluate for infection/inflammation etc.
