Question: Hi,
My father is a diabetic and hypertension patient ,he is under medications for the same.Recently we met our nephrologist and got an ultrasound scan of his kidneys.Hereby i have attached the recent blood test report with this and the prescription given by the doctor. But recently he has got seVre pain in his joints of his feet, leg and hands.I doubt it's due to the uric acid levels that are high in his blood. and it may be a gout attack.I can't understand why the doctor hasn't prescribed any medications to treat his gout pain that he s is suffering. He has asked to take a blood test report again. Please help with your suggestions.

Detailed Answer:
Hello,
Welcome and thanks for your query.
I have reviewed your father's results and medications. He has moderate renal failure and his uric acid elevation may be related to the same.
The medicine Zurig prescribed by his doctor is to reduce the uric acid level.
His boney pains are likely to be due to Vitamin D3 deficiency. Vitamin D3 has to be metabolised in the liver and kidney to be the active 1,25,D3. If he is found to be deficient, then he has to be treated with 1alpha to get relief.

Detailed Answer:
It is moderate renal failure, not severe.
Proper control of diabetes, avoiding infections and medicines like NSAIDs that can harm the kidneys further have to be avoided.
Swelling may be related to gout but is less likely if many joints are involved.
One way to check would be to use colchicine. If it is due to gout, the relief will be quick.
Better to get vitamin D level checked also.

Brief Answer:
Possible , discussion follows

Detailed Answer:
There's treatment for gouty arthritis. To decrease the inflammation and pain Colchicine is probably the best and doesn't harm the kidneys unlike NSAIDs. The uric acid levels can be reduced by Xanthine oxidase inhibitor, your grandfather is already on one. His doctor can adjust the dose. When the uric acid brought to normal levels, the chances of further episodes are less.
