Many Viridans strep-like colonies (A)
Many Coryneform bacteria (A)
Few Lactose Negative Gram Negative Rod (A)
Few Lactose Positive Gram Negative Rod (A)
Stain
Many Gram positive cocci in pairs and chains
Moderate Pleomorphic Gram Positive Bacilli
Few WBC
Few Epithelial Cells
May suggest - infection by multi-bacteria
Detailed Answer:
Hi,thank you for posting your query on health care magic.
The characteristics of your culture report will suggest possibility of infecting bacteria as follows:
“Lactose-positive gram negative rods” may suggest Enterobacteriaceae, such as E. coli, Klebsiella, or Enterobacter species.
Serratia species are gram-negative bacteria classified in the tribe Klebsielleae and the large family Enterobacteriaceae.
Patients with Serratia sepsis may present with Urinary tract infection,Respiratory tract infection,Intra-abdominal infections,Ocular infections,Soft-tissue infections,Otitis media, etc
Viridans streptococci: viridans group streptococci (VGS) are known to colonize human respiratory, gastrointestinal, and genitourinary tracts. These bacteria are pathogenic given the right conditions.
C diphtheriae infection is typically characterized by a local inflammation(low-grade fever, sore throat, nausea, vomiting enlarged lymph nodes, severe swelling in neck, pseudomembrane - greenish-gray solidified fibrous exudate cells) usually in the upper respiratory tract, associated with toxin-mediated cardiac and neural disease.
“Lactose-negative gram negative rods” may suggest Pseudomonas.
The most common gram-positive cocci that form pairs include most Streptococcus species, Listeria and Enterococcus bacteria.
Pleomorphic gram positive bacilli-suggestive of Diphtherial infection.
Looking at the above features, you might be suffering from an upper respiratory tract infection due to Diphtheria and other bacteria like Pseudomonas,Sterptococcus and Serratia.
Are you suffering from symptoms of upper respiratory tract infection?
You might be needed to be treated with : diphtheria antitoxin, combination of antibiotic- antipseudomonal beta-lactam (eg, penicillin ) and an aminoglycoside .
Hope this answers your question.I would be happy to answer if you have any follow-up questions.
I do have very strained neck and shoulders to the point of not being able to quite turn at this point. However my throat does feel better at this point compared to when the test was taken but not to the extent of 100 percent at all yet after taking Augmentine for 14 days compared to when the test wa taken.
What course of action would you recommend ? Should I be taking another culture and gram stain test again? And would you think the strained muscles could be related to this at all and if its indicative of a different problem?
Thanks in advance,
XXXX
Need to consult an Otolaryngologist for throat examination
Detailed Answer:
Hi XXXX,thank you for your reply.
Neck stiffness is often caused by three types of problems: muscle strain, cervical spine disorders, or (less commonly) an infection. An infection is not a common cause of neck stiffness, but it is a potentially serious condition so it is worth noting.
Any activity that places your neck in an awkward position for an extended amount of time could cause the muscles around in your neck to become fatigued and spasm.
The cervical spine encompasses all of the bones, joints, muscles, and nerves in your neck. Any one or combination of these parts of your cervical spine may become worn down over time or injured, causing neck stiffness. In such a case ,you might feel pain, numbness, discomfort, and weakness-inability to move or use arms, etc. If you have such symptoms, then it would be advisable to consult an Orthopedic doctor.
A stiff neck, in conjunction with a high fever, headache, nausea or vomiting, sleepiness, and other symptoms, may be indicative of meningitis, a bacterial infection. If this is suspected, then you need to consult a Physician doctor and CSF Fluid Analysis and culture is required.
Looking at your symptoms, you might be suffering from non-diphtheria Corynebacterium infection.Antibiotics are the treatment of choice for nondiphtherial corynebacteria infections. As answered previously, you might not be suffering from Diphtheria Corynebacterium infection as if infected you might be suffering from other symptoms like nasal discharge, enlarged lymph nodes in the neck, membrane in the pharynx, etc and you need to take Diphtherial Antitoxin along with Antibiotic.
You have taken Antibiotic Augmentin(Amoxycillin with Clavulanic Acid). It is effective against most of the suspected bacteria like Non-Diphtherial Corynebacterium,Streptococci, etc.but might not be effective against other suspected bacteria-pseudomonas.Fluoroquinolone group of antibiotic like Levofloxacin might be effective in this case.
As still you are not completely relieved of throat pain,it would be advisable to consult an Otolaryngologist doctor .After doing throat examination,it can be decided whether a repeat culture and gram stain is needed or not.
Hope this answers your question.Get well soon.
