Brief Answer:
Polycystic ovarian syndome

Detailed Answer:
Hi dear, I have gone through your and your husband's reports.
You serum insulin levels are raised and the ultrasound shows multiple small follicles in both the ovaries. These findings with prolonged cycles are suggestive of Polycystic ovarian syndrome (PCOS). This is a hormonal disorder, leading to irregular or no ovulation at all, and hence difficulty in conception. The reports also show decreased vitamin D3 levels, for which you should take vitamin D3 supplementation.
Your husband's semen analysis shows 45% non motile sperms and presence of WBCs indicative of infection, and this also cause decreased fertility. The semen culture shows the presence of bacteria which is sensitive to ofloxacin and levofloxacin. He should take either of the two to treat the infection.
